The project applies a holistic population health management perspective with the lens of equity by integrating the PCMH (person-centered medical home) model and SDoH (social determinants of health) model in a way that promotes health equity and better addresses health disparities.

Responsibilities:

As a Backend Developer, you serve as a team expert in implementation, actively participate in product features and design discussions to help shape product.

Write clean and maintainable code, adhering to best practices and coding standards, to ensure the scalability and performance of applications.

Troubleshoot and debug issues, ensuring the timely resolution of bugs and optimal functionality of the applications.

Work closely with the team to deliver high-quality solutions aligned with project requirements and timelines.
